Submit Your Request

  • PTO membership meetings are held on the 2nd Thursday of each month in the Indian Head library, unless otherwise notified. 
  • Teacher requests must be submitted to the PTO mailbox 7 days prior to the schedule PTO membership meeting.  Requests received after that will be put on the ballot of the next meeting.
  • Order forms & essential information must be submitted with request form.  Costs must be current and include shipping & handling, if applicable.  
  • Principal signature is required.
  • Request forms are available in the front office.
